We look forward to providing you with top-quality pain management care. On your initial visit, you will meet with our highly qualified, double board-certified pain management physicians. After a thorough exam and review of your medical history, our physicians will develop a comprehensive treatment regimen to help manage and improve your pain. Our goal is to restore your function and improve your pain through medications, minimally invasive treatments, and the most novel medical approaches to pain management.

Our doctors focus on advanced interventional pain, so our office can treat nearly any kind of pain. After a thorough examination, your doctor will recommend the pain treatment option that he or she believes will best treat your pain condition. Please view our list of traditional, as well as cutting-edge procedures, we offer to our patients. You will find a complete list of procedures, pain information, and conditions related to each option.
Please obtain copies of your records such as any imaging, previous procedure/surgeries, previous pain management treatments, etc. We have included an authorization for medical record release for your convenience. You may then give it to your referring physician to expedite the process. Please bring any reports of old imaging that has been done.
For all of our pain clinics, we attempt to confirm all new patient appointments 24 hours in advance. If you cannot keep the appointment on the attached card, please notify us within 48 hours of your appointment time. You can also cancel or change any appointment directly through the patient portal.
We do offer IV sedation for all procedures performed by Dr. Seth or other providers at our pain management clinic. This is administered by a CRNA who works with Dr. Seth throughout the entire procedure to ensure your safety and comfort. To receive sedation, you must follow the required steps:
Following your pain treatment procedure, you will be discharged the same day with your post-procedural instructions. Due to the local anesthetic that is used, it is common for patients to experience weakness and or numbness in the treated areas for a few hours after the procedure. Within the first 24 hours of your procedure, you may experience an increase in your pain due to inflammation from any steroid that is used. This should subside in a short time and can usually be controlled with anti-inflammatories and ice. If you experience any of the following symptoms, please contact us immediately:
You will be required to check in 15 minutes prior to your scheduled pain management appointment time to fill out the consent forms for the procedure. All copays, coinsurance, and deductible payments are due at the time of service, unless other arrangements, such as a payment plan, are made in advance. If you need to cancel or reschedule your procedure please notify us within 24 hours of your scheduled time to avoid any cancellation fees. We also ask that you bring a list of your current medications, allergies, and medical conditions with you on your procedure day. We ask that our patients wear loose fitting clothes to their procedure so the providers have access to the area that they are treating.
If you intend to use insurance at our pain clinic, please provide our staff with your insurance information card. If your insurance changes, prompt notification to the staff will ensure accurate billing. See the insurance companies we work with and please contact us if you don’t see yours listed as it evolves and we might work with your carrier.
For pain clinic appointments, patients are requested to arrive 15 minutes before their scheduled appointment time. If you are being seen by one of our providers for the first time, please arrive an additional 30 minutes early to ensure that you have adequate time to complete our new patient intake paperwork. If you prefer, you can download, print, and complete this paperwork prior to your visit.
For procedures, patients need to arrive 30 minutes before their scheduled procedure time. It takes some time to prepare for your procedure including check-in, changing, blood work, and IVs. We want to make sure we have enough time to give you truly great care.
If your insurance requires an authorized referral for care at our pain management practice, please secure this referral from your primary physician prior to your arrival at the office. Your visit cannot begin until a referral is received.

Yes, we can help if you’re experiencing pain as result of a workplace injury. Worker’s compensation patient and patients working with a lawyer need to have the appropriate information at the time of their appointment including case number, law firm information, address and contact number for claims, etc.
